First, please contact the General My Number Toll-Free Number (0120-95-0178; available 24 hours a day, 365 days a year), which can temporarily suspend use of your card.
If your My Number (Individual Number) remains unchanged, your My Number Card can be reissued in one week. Please consult with the municipality in which you live.
If you have an appointment at a medical facility before your My Number Card is reissued, Astellas Health Insurance Society will issue a Health Insurance Eligibility Certificate, so please immediately submit the Health Insurance Eligibility Certificate (Re)issuance Application Form through your company’s personnel affairs staff.
If there is risk of unauthorized use due to theft, etc. and your My Number has changed, please submit the Notification of Change to the Health Insurance Identification Number (My Number) through your company’s personnel affairs staff. Astellas Health Insurance Society will register the new My Number so that you are able to receive medical care at medical facilities.
