I lost my Health Insurance Eligibility Certificate and would like it to be reissued. What should I do?

If you lose your Health Insurance Eligibility Certificate while it is still valid, after filing a Lost Property Report at the nearest police station, immediately submit a Declaration of Lost Health Insurance Eligibility Certificate to your company.
If you need your Health Insurance Eligibility Certificate to be reissued, please submit the Health Insurance Eligibility Certificate (Re)issuance Application Form through your company’s personnel affairs staff. A fee of 2,000 yen will be charged to reissue the Eligibility Certificate. Note that if you have registered your My Number Health Insurance Card, you do not need to submit the Health Insurance Eligibility Certificate (Re)issuance Application Form.
