If you are able to view the Medical Insurance Eligibility Information page on the Mynaportal website, then applying for reissuance of the Health Insurance Eligibility Information Notice is generally not required since displaying the webpage can be used as a substitute.
If it is necessary to reissue the Health Insurance Eligibility Information Notice, please submit the Health Insurance Eligibility Information Notice Reissuance Application Form to your company.
