When I received medical care at a medical facility, I was told that the address on my My Number Health Insurance Card and address on the medical facility’s medical questionnaire differ. Why is that?

Due to system specifications, the address used by Astellas Health Insurance Society may differ from the My Number Health Insurance Card address since it shows the address where the insured individual resides. However, even if a different address is shown, you can still receive medical care at medical facilities. If your address is checked by a medical facility when receiving medical care in the future, please explain that due to the system used by the health insurance society, the insured individual’s address is shown.
