1. Overview of the Transition to My Number Health Insurance Cards

Future Schedule

Period During Which Conventional Health Insurance Cards Can Be Used (Transitional Period)

  • After December 2, 2024, the issuance and reissuance of conventional health insurance cards will be abolished, and visits to medical institutions will require the use of My Number health insurance cards.
    Those who possess valid health insurance cards before the abolition date may continue to use them during the transitional period until December 1, 2025, after which they will no longer be usable.
    • * Health insurance cards were issued for applications that reached our Health Insurance Society via the company by Thursday, November 28, 2024 (only if there were no problems or omissions).
  • For individuals who have not obtained a My Number card or have not registered their My Number card for use as a health insurance card (My Number health insurance card), our Health Insurance Society will verify and issue a Qualification Certificate by December 1, 2025 (scheduled for around summer) during the transitional period (individuals are not required to apply).

Method
When individuals with a My Number health insurance card visit a medical institution, if the medical institution does not have a card reader or it cannot be used due to a malfunction, present the Eligibility Information Notice together with your My Number health insurance card. When individuals who have not obtained a My Number Card or have not registered their My Number Card as a health insurance card (My Number health insurance card) visit a medical institution, this is presented and used instead of a My Number health insurance card.

Remarks Although not strictly necessary for those without a My Number health insurance card, it is issued in anticipation of a My Number health insurance card being created in the future. 2. Eligibility Information Notice

Issuance of Eligibility Information Notice

  • For all employees who join the company after the abolition date of health insurance cards (new issuance and reissuance) and all individuals newly certified as dependents of the Health Insurance Society, the Health Insurance Society will complete a procedure called Online Eligibility Verification that enables medical institution visits.
  • After the above procedure is completed, an Eligibility Information Notice will be issued and delivered via the company. Once you receive it, you can visit medical institutions using your My Number health insurance card.
    • * If there are problems with the application form details or any missing documents, data registration for Online Eligibility Verification cannot be completed, and issuance of the Eligibility Information Notice will be delayed. For this reason, please ensure that documents submitted to the Health Insurance Society include accurate personal identification numbers (My Number) and the name (kanji and kana), date of birth, gender, and address listed on your resident registration or My Number card.

Using the Eligibility Information Notice

  • You can confirm your health insurance code and number, which are needed for various applications to the Health Insurance Society and inquiries.
  • When individuals with My Number health insurance cards visit medical institutions, if the medical institution does not have a card reader for My Number cards or if the card reader is not functioning properly, you can receive insurance-covered treatment by presenting the Eligibility Information Notice together with your My Number health insurance card.
    • * You cannot visit medical institutions with only the Eligibility Information Notice.

If You Lose Your Eligibility Information Notice

  • If you wish to have your Eligibility Information Notice reissued, please submit a Health Insurance Eligibility Certificate (Re)issuance Application Form. However, if you can access the Medical Insurance Eligibility Information Screen on Mynaportal, this application is generally not required, as the screen can be used as a substitute.

3. Qualification Certificate

Issuance of Qualification Certificate

  • For those who have not obtained a My Number Card or have not registered their My Number Card as a health insurance card (My Number health insurance card), the Health Insurance Society will verify their status and issue a Qualification Certificate to eligible individuals, which will be delivered through the company.
  • The Qualification Certificate will be valid until December 1, 2029, so please switch to a My Number health insurance card in accordance with national policy before that date.

Using the Qualification Certificate

  • Those who do not have a My Number Card or who have not registered their My Number Card as a health insurance card (My Number health insurance card) can present the Eligibility Confirmation Certificate instead of a My Number health insurance card when visiting medical institutions to receive insured medical treatment.

If You Lose Your Qualification Certificate

  • If you lose your Qualification Certificate, please submit a lost property report at a police box or police station.
  • If you wish to have the Qualification Certificate reissued, please submit a Health Insurance Eligibility Certificate (Re)issuance Application Form. Please note that a reissuance fee of 2,000 yen will be required, so keep the certificate safe and be careful when using it to avoid losing it.

Returning the Qualification Certificate

  • If you lose eligibility before the expiration date of the Qualification Certificate (December 1, 2029) due to leaving the company or a dependent being removed from health insurance coverage, please return the Qualification Certificate to the company.

4. Conventional Health Insurance Cards

If You Lose Your Conventional Health Insurance Card

  • If you lose your health insurance card after December 2, 2024, please submit a lost property report at a police box or police station, then submit a Declaration of Lost Insurance Card.
  • Conventional health insurance cards will not be reissued, so please use your My Number health insurance card (or Qualification Certificate).

Returning Your Conventional Health Insurance Card

  • If you lose eligibility before the end of the transitional period for health insurance card use (December 1, 2025) due to leaving the company or a dependent being removed from health insurance coverage, please return the health insurance card to the company.
  • After the end of the transitional period, it is not necessary to return the health insurance card; please dispose of it yourself.

My Number System and My Number Card

Using Your My Number Health Insurance Card

STEP1 Submit your Individual Number (My Number) to the Health Insurance Society through your company when joining the company or enrolling dependents in health insurance.
STEP2 Applying for and obtaining a My Number Card individually:
Please check with your local municipality regarding application methods.
(The Health Insurance Society cannot register My Number health insurance card usage)
  • * Simply having a My Number Card issued does not enable it to be used as a health insurance card.
STEP3 Performing My Number Health Insurance Card Usage Registration yourself (via Mynaportal, Seven Bank ATMs, card readers at medical institutions, etc.):

My Number Card Electronic Certificate

  • The electronic certificate written on the My Number Card has a validity period of five years (until the fifth birthday from the date the electronic certificate is issued). For those whose validity period is approaching, the Ministry of Internal Affairs and Communications/Japan Agency for Local Authority Information Systems will send a Validity Period Notification approximately two to three months before the expiration date with information about the renewal procedure, so please complete the procedure.

  • If the validity period expires, you will no longer be able to log in to Mynaportal, and the card can no longer be used for electronic applications such as e-Tax, convenience store certificate issuance, or as a health insurance card. Please complete renewal procedures at your local municipal office. For details, please contact your municipal office.

If You Lose Your My Number Card

  • Contact the Individual Number Card Call Center (0120-95-0178) to perform temporary suspension procedures. If you lose your My Number Card outdoors, please submit a lost property report at your nearest police box or police station before applying for reissuance. When applying for reissuance, you will need the police station name, telephone number, officer's name, and lost property report acceptance number, so please make note of these in advance.
  • If your Individual Number changes when your My Number Card is reissued, please promptly notify your company and the Health Insurance Society.

6. Individuals Moving Overseas

For those who returned their My Number Card when moving overseas and those who have not yet created a My Number Card, you may either use your conventional health insurance card until December 1, 2025, or use the Qualification Certificate that will serve as a substitute for your health insurance card, which will be distributed through your employer by December 1, 2025. Please use this when receiving medical care during temporary returns to Japan, etc.

How to Switch to a My Number Card for Overseas Residents Before Departing Japan

If you are planning to move overseas and currently hold a valid My Number Card, you can continue to use your My Number Card after leaving Japan by completing the following procedures by the day before your scheduled departure date. How to Apply for a My Number Card After Moving Overseas

Those currently living overseas without a My Number Card (limited to those who moved overseas after October 5, 2015) can now apply for a My Number Card through overseas diplomatic missions or the Overseas Application Form Download Page. We recommend creating a My Number health insurance card to enable prompt medical care during temporary returns to Japan or when returning permanently. For Those Who Moved Overseas Before October 5, 2015

  • Those who moved overseas before October 5, 2015, when the My Number System began
  • Children born overseas after October 5, 2015 who have never been registered as residents in Japan

For these individuals, a My Number has not been assigned, so they cannot apply for a My Number Card overseas. Those who fall into these categories can apply for a My Number Card only after moving to Japan, registering as residents, and being assigned a My Number.
For this reason, until you move to Japan, you may either use your conventional health insurance card until December 1, 2025, or use the Qualification Certificate that will serve as a substitute for your health insurance card, which will be distributed through your employer by December 1, 2025.

After moving to Japan and being assigned a My Number, please contact your company, as it will be necessary to link your My Number with your health insurance code and number.
