We are conducting a Periodontal Disease Risk Test for early prevention and to prevent severe cases of periodontal disease from developing.

Eligibility: Insured individuals who are 30 years old (those who will be 30 years old by the end of March of the current fiscal year)
No application is necessary. A test kit will be sent to your home. (Shipping starts around mid-September)
